Co-Pay Payment
If you are coming for electrodiagnostic testing (EMG/NCS), please do not wear lotions or jewelry (wedding rings are okay).
Please let us know if you have an IMPLANTED CARDIAC DEFIBRILLATOR ahead of your appointment, as this will affect your testing.
Co-payment is expected at the time of service.
We accept most major insurances. Please contact us should you have any questions.
We accept cash, checks, and major credit cards. Any amounts not covered by your insurance will be your responsibility.
If you do not have health insurance and are a "self-pay" patient, we will provide you with an estimate of charges before you are seen. Payment is due in FULL at the time of service.
Workers compensation and auto insurance: If we bill workers compensation or auto insurance, we need the following information: insurance company name, address, claim number, and contact person. If workers compensation or auto insurance determines that your visit was not authorized, YOU will be responsible for payment either with personal insurance or self payment.
We will not release information about your care without written authorization. You will be asked to review and sign a privacy policy notice.
If you are able, please fill out the Welcome Form and the Contact and Insurance Form before your visit.
To access the forms, click on the above links. The forms can be filled out on the computer or printed then filled out. If filling out on the computer, you will need to print and sign the forms after the fields are completed.
